The Borough has 4 integrated health and social care teams which are well established professional teams who work with residents to:

•Promote choice and control through personal budgets and self directed support
•Improve the service user experience through joint working with health colleagues
•Safeguard people from abuse
•Support independence through offering support at home

The talented staff work closely with service users and their families to ensure that the service user is at the centre of all work undertaken by the team. They also have very good partnership working relationships with professionals in primary health care, specialist health care and the voluntary sector to ensure a holistic approach to supporting residents.

The Assistant Team Manager supports the Team Manager in leading and managing an integrated health and social care team. The incumbent is part of the management team and responsible for ensuring continuity of social care services in the locality, and that provision is outcome focused, that needs of service users are met and that the use of resources is maximised.
